Thanks, very sad story. Glad he survived. This sounds like an anchor accident, or, "anchor accident with intent to rappel." I wrote about it, here,

Whoa Sean, that's some epic reading, amazing photos and of course glad you made it out alive.

After reading the full report, I have a foggy recollection of hearing you describe the incident on a podcast (The Sharp End)? I've followed your adventures for years, glad to know there will be more to come.
